Form look great no error message when filled in , email arrives with in seconds, information looks like this.

It is most likely being caused by your email client. I would personally try checking your mail through a different means.

Also, try changing the format that messages are sent in.

1. Login
2. Click 'Edit'
3. Click 'Basic Form Information'
4. Change the format to 'Text'
5. Click 'Submit Changes' button
