Hi, I set-up this form at: http://www.medspa360.com/freeconsultation.htm and when you fill-in all the required fields, "Full Name" keeps popping up with an error message saying that the field needs to be completed, which I did. I can't figure out what I'm doing wrong.

1. Recopy and paste the html
2. If #1 doesn't work, delete the question causing the problem and recreate it.
3. If #2 doesn't work, delete the form and rebuild it.
