Try to edit "E-mail Address" field. Change the selection for display to "horizontal" from "vertical," it displays the word "horizontal,' but it does not change. The entire form is displayed vertically, even though all other fields are indicated as horizontal.

It is changing in the code, but likely not working if the width of the form or the location it is being placed is too narrow and causing the field to wrap to the next line. Try increasing the width of your form.
