I have reviewed this form many times but it is not working. The form is located here:

on the contact page. I have set it up to work at

I have looked through this and tested it countless times. What could be wrong?

Unfortunately, the form is not working based on anything on the myContactForm.com side or config. There is additional CSS/Javascript on your page which is preventing forms from working.
