We received the "Error: Upgrade Your Account" message; however, the person that owned the account is no longer an employee, so we cannot login to upgrade it. Is there a way to transfer ownership?

The account login is the same as the forum login. You can change the details associated with the account under the My User Information portion of the main control panel. You can upgrade the account under the Premium Membership portion of the control panel.

We don't have the login user name or password for the ex-employee that maintained our account.

How are you logging in to this forum? Use the same user and password to login to your account. Ownership info can only be verified and changed within the admin panel.

You can upgrade the account directly without a login, here:

We created a separate account just to post these questions to the forum. We do not have the login user name or password for the account that needs to be upgraded.

Please email me (info@mycontactform.com) the URL of the account. I can look up what emails are listed as administrators for the account. Unfortunately, I can only send lost password information to the email addresses listed as administrators within the account. If you do not have access to these email addresses, you may need to re-create your form within your new account and update the code on your site.
