I am unable to proceed past step 5 of 14 "Form Questions"

I input required information such as: width 30, characters allowed 30, initial value blank, required "yes", layout horizontal.
I click on next...then...nothing the page does not change or move forward. Am I doing somthing wrong? Seems simple enough.

1. Logout of your browser
2. Close the window
3. Open a new window
4. Login to the site
5. Try again

It could get hung up if you are using single or double quotes in the question/answer.
