No matter what the person enters, in the following checkboxes I only get the checkbox27928
AM/PM;Scene Location;ME Case;Sex;Type of Call; Provided.
Some are single and others are multi check. I NEED HELP ASAP. Thanks, Sam Carlson

1. Recopy and paste the code to your site and see if this resolves the problem.
2. Please post your username and form location so we can further investigate.
