My form location is: https://www.lasvegasmedicalresearchcenter.com/alzheimers-questionnaire

When I test my form, I am getting an error message that says I need to change the form location to:

lasvegasmedicalresearchcenter-com.filesusr.com/html/be09e2_e0f7fb070f551f0ebe2b2ff98948f299.htm

But the forms manager is saying this is not a secure locationa nd won't let me use that link.

Can anyone assist please

Try adding https:// to the beginning of the form location setting.

This URL is the actual location that your server is communicating to our script. You're likely using an iframe to display the form via the templating / CMS that you've selected.
