Log in| Every form I create is not responding |

When I create a form I can load it on the html and it shows up fine. But when I test it and hit submit it just reopens the same page. When the page opens the fields are blank. I have created several forms before with out problems. I have tested the forms on my other sites and the new forms I test are not working either.
