I copied and pasted EXACTLY what the html says to my website (www.bradleycleaning.com/estimates.html). I haven't changed anything, and it says this message:

Error 404: Page Not Found

I am not sure what is going on here. My webhost is Bravenet.com so i don't know what is going on. I am going to paste my html code that this site gave me.

The problem is that either you our your host changed the code. For the action portion of the html code, it is showing up on your site as:
