Our premier account was suspended when a new employee created a form that included a field for social security numbers. The employee was unaware this was a violation. We removed the field on the form. We have submitted a request to have the account reinstated. Would you please reinstate our account? All our other forms, which are compliant, are not working and we are right in the middle of a campaign. We have fixed the problem, can you please reinstate the account.
