hi guys
I've managed to create my form but only half a border surrounds the form. It is on the home page of my site which is www.successfulbetting.co.uk
Also now that I'm a premium member does powered by Mycontactform stay visible.
Many thank for your help

It looks like the border is being clipped by the other HTML around it. I would try shrinking the form or modifying the page layout to accommodate the form better.
Once upgraded, the link back to myContactForm.com is automatically removed from the code you copy from the admin panel. In order for this to be reflected on your live site, you need to update the code.

This is likely an issue with the CSS / HTML surrounding the form in your page template. I would start there to figure out why it isn't showing. It is also possible that you've inserted the form HTML into an iframe that is inadequately sized and clipping off the bottom portion of the form.
