I do not have a web page but would like to use mycontactform.com in an email. How do I get that link?

Premium members can be provided with a 'hosted form'. This is basically a URL that you are provided with that has a copy of the fully-functional, publicly accessible form. It is found under Xtras > Hosted Form.
