I was just checking my form to make sure that it worked and it keeps saying--- We're sorry but you did not fill out all of the required fields. Please push the back button on your browser and fill out all of the required fields.

I know I am putting the phone numbers in everywhere because I have checked several times to make sure. Is there something I can do to fix this?

Try deleting the phone number question from your form, re-adding it, and then updating the code.
