Hi, I have a form here: http://www.magickitchen.com/catalog.html

My boss would like to receive the information from the form without the questions. That way, he can just copy and paste the information without the questions getting in the way.

Currently we get this:

Your Name: Kenneth Beeble
Please enter complete mailing address here: Kenneth Beebe
7622 Southwest Londaugh St
Portland, OR 97229

We'd like to get:
Kenneth Beebe
7675 Southwest Londaugh St
Portland, OR 97223
