Hi,

From the basic form information!

What format would you like the e-mails sent in?
Plain

Include empty question responses in your e-mailed results?
YES

Include the web address of form in results?
YES

The form arrives with all fields but not the web address it is on.

Good Day,

For some form submitters, their internet provider or web browser will block this information. When this occurs, the URL will not be able to be shown in the form results.
