You can configure your form for multiple different domain / web pages by:
1. Logging in to myContactForm.com.
2. Click edit next to your form's name.
3. Click Basic Form Information
4. Enter the various URLs where you will be using the form under Item #2: What specific web address(es) will this form be located (include filename)?
Make sure to include only one URL per line.
